Integrative Physiotherapy is based in Covingham, Swindon, and is run by Specialist Pain Physiotherapist, Nicola Brown.
The clinic specialises in improving the lives of people with a range of long-term and complex conditions. We offer pain specialist physiotherapy, pain rehabilitation, health coaching, nutritional therapy, and acupuncture, or a combination of these, to achieve this.
We work together to improve your pain, fatigue, mobility, and a range of other symptoms, so you can feel healthier, more energised, and life your best life.
Our way of working is for you if you are interested in taking a more holistic approach to your health, and to feel the many benefits that this can bring.
We support you by developing an individualised treatment programme, unique to your needs, bringing together a wide range of techniques and practices and supporting you all the way.

I am passionate about working with people living with persistent pain, and my role is to have a broad and deep knowledge of cutting-edge pain science and to interpret this for clients, so that they can achieve their best results.

Why make time to exercise when you live with ongoing pain?
Although it isn’t always easy to motivate ourselves to move and be more active, especially when pain is present, there are such a wide range of benefits.
If it were a pill, we would all be taking it!!
Exercise/Movement:
- Regulates sleep and reduces fatigue.
- Reduces inflammation.
- Improves mood and reduces anxiety.
- Increases brain size and improves memory, concentration and decision making.
- Reduces the risk of developing diabetes, having a stroke, depression and anxiety,
many types of cancer, and dementia.
- Helps support the immune system.
- Improves resilience to stress.
- Strengthens muscles reduces the stiffness in joints.
- Improves bone density, digestion, heart health, circulation
There is also good evidence that exercise helps with both pain and function (what we are able to do) in conditions such as back pain, arthritis, fibromyalgia, and cancer.

The Mediterranean diet has been extensively studied and found to improve overall health and reduce low grade inflammation (which we know impacts on pain and has been discussed in a previous post).
It can also aid weight management, improve cardiovascular function, and post-operative/muscle recovery.
If you’re confused what it actually contains though, take a look at the list below:
- Healthy fats such as extra virgin olive oil and avocados.
- Low intake of red meat and processed foods.
- High plant foods, such as, fruits, vegetables, nuts, seeds, beans, and whole grains (e.g. oats, bulgur wheat, quinoa, whole grain rice and pasta).
- Moderate lean protein, such as salmon, sardines and chicken.
- Moderate dairy, especially yoghurt and cheese.

“I hate cold water”.
I can’t remember the last time I went into the sea and even avoided swimming pools, if I felt they weren’t warm enough.
I was reading more about the benefits of cold water therapy on the immune system, how it improves your mood, energy and reduces stress.
At the same time, my mum invited me to an open water induction at the local lakes 😩
I could have said no, stayed firm to my belief, and in what we call a fixed mindset (that we cannot change).
Instead, and thankfully, I went for it and with each swim felt more energised, acclimatised to the cold, and found myself actively looking forward to the next one.
This is a growth mindset - the belief that we can, through perseverance, practise, and effort, improve and develop our skills.
This same mindset can apply to pain, to the things we avoid, or tell ourselves we can’t do anymore.
This isn’t a magic bullet but what if you asked instead, what can I do?
Could I do it differently? Where could I start?

One of the factors from the spider diagram last week that can contribute to pain is low-grade inflammation, and I wanted to explain this in more detail.
Whilst inflammation itself is not bad, that tell tale redness and swelling after you've stubbed your toe or sprained your ankle is a really important part of the healing process.
Having an immune response that goes on for months, if not years, is no longer helpful and actually now shown to be harmful to be body in many ways.
It makes us age faster and been shown to contribute to every chronic condition we can think of, such as diabetes, dementia, cancer and heart disease.
In terms of pain, it leads to neuro-inflammation, which causes a sensitisation of the nervous system, reduces the effect of our own endorphins, overall resulting in increased pain.
Poor sleep, stress, being sedentary, smoking, alcohol, ultra-processed foods, and pollutants can all lead to low-grade inflammation, so making lifestyle changes can make a real difference.

I've put together this spider diagram to illustrate the many factors that impact our experience of pain.
We know that tissue damage and injury can play a part but they are certainly not the whole story!
This is a simplified version of the things we know can impact pain, and each box could be expanded again into a more complex web.
For each person that I see in clinic, the size/impact of each factor will be different, and part of my role is to work out which of these are having the most impact.
There are things that we can't change, like our genetics, but we can influence the way that our genes are 'expressed' or function, through a range of lifestyle changes.
